Collage Home and Community offers comprehensive, intensive home and community-based neuro-rehabilitation services to adults and children with mild, moderate and severe acquired brain injuries. Collage is dedicated to providing person-centered care that is meaningful, evidence-based and functional. Emphasis is on real-life rehabilitation that maximizes independence and quality of life. Treatment occurs in the individuals own home and neighborhood community, improving carryover and learning of skills and strategies. Outcomes that last are achieved effectively through an integrated team approach led by a case manager. Our programming is tailored to address the individual's needs regardless of their stage in the rehabilitation process. The individual's experienced team of neuro-rehab specialists may include physical, occupational, speech, cognitive and recreational therapy with counseling, neuropsychology, social work and behavior analyst support as needed.
