SLP Colorado’s Mission: To provide long term community based residential services to individuals diagnosed with a traumatic brain injury by supporting the growth of maximum independence in the least restrictive environment.

SLP Colorado’s Residential Supported Living program is designed for individuals who are living alone or with family, or may have completed a post-acute or neurobehavioral rehabilitation program and need a structured living environment to support and improve their level of functioning.

SLP Colorado offers a community based residential setting for adults in Parker, Co. Our home is located in a wonderful neighborhood, backs to open space, trails and a park. Services and activities are convenient for our residents, allowing easy access to their community. SLP provides the resources and opportunities, necessary, for each resident to live a diverse, full life.

Abundant Blessings and Care Assisted Living

Assisted living facilities are for people needing assistance with Activities of Daily Living (ADLs) but wish to live as independently as possible for as long as possible. Assisted living exists to bridge the gap between independent living and nursing homes. Residents in assisted living centers are not able to live by themselves but do not require constant care either. ARA includes companies to provide residential, vocational, counseling, nationwide support and business solutions in Wisconsin and Colorado. Brain Care

BIAC Partner

BrainCare provides daily support for individuals with acquired brain injury (ABI), including traumatic brain injury (TBI). Through a range of services—including specialized living communities, coordinated care, and cognitive rehabilitation with skills training—we help participants relearn lost skills and grow towards independence and self-sufficiency. Our residential options are designed in a continuum fashion with the goal of helping the person move to greater independence. These include an assisted living community and three 8-person houses. The Lodge offers services for individuals that require higher levels of skilled care, and the homes are tailored to meet the needs of each individual. BrainCare’s focus is not just to help our participants recover, but thrive after a brain injury.

Greeley Center for Independence Brain Injury Services

Campus style Medicaid certified Supported Living Program for adults with brain injury and Specialized Adult Day Services. Eighteen efficiency apartments with 24/7 supervision and support from onsite staff including CNAs, personal care providers and life skill trainers. Day Program 5x/week with social and recreational activities for all ability levels. Greeley Center for Independence, Inc (GCI) is a non-profit agency devoted to improving the lives of people with disabilities. Stephens Brain Injury Campus is a certified provider of services under the Medicaid BI-HCBS Waiver.
